Conducting regular hazard evaluations at work is another foundational aspect of controlling and preventing hazards in the workplace that cause accidents. Job Hazard Analysis (also commonly referred to as “Job Safety Analysis”) is a recommended method by OSHA for evaluating hazards associated with specific job tasks. This assignment provides an opportunity for you to practice conducting a Job Hazard Analysis.
This assignment draws from all of the learning materials in Week 2 and the Chapter 6 in the textbook from Week 1.
Using the Job Hazard Analysis form linked here , in the designated columns, document:

- 3 potential hazards (could cause an accident) OR
- 3 existing hazards (will cause an accident) OR
- 3 previous hazard s (could have or did cause an accident but were corrected) OR
- A combination of any 3 of these types of hazards in your current or a previous workplace.
- Also document the specific job tasks where these hazards exist AND
- Your recommendation on controlling the hazards AND
- Any Personal Protective Equipment (PPE) that is required to or helpful in controlling the hazard. If PPE is not needed for that hazard, put N/A in that box.
- For your recommendations that are not PPE, use the NIOSH Hierarchy of Controls from this week’s learning materials and include which other level of control your recommendations is. (PPE is a level of control itself)
